Who this is for, and when it’s required
You need a GFE if the client is:
- Uninsured, or paying out of pocket.
- Insured, but choosing not to submit a claim for these sessions.
- Scheduling an appointment, not just asking about your rates.
The delivery timing is specific:
- Appointment scheduled 3+ business days out: estimate owed 1 business day before.
- Appointment scheduled 10+ business days out: estimate owed 3 business days before.
- Recurring care over 12 months: one estimate can cover the full course, reissued yearly.
If a client is later billed at least $400 more than their estimate for the same care, they can dispute the charge. Providers who don’t provide a required estimate can face civil penalties of up to $10,000 per violation. Read the full breakdown, including citations, in the docs.
